$78,000-82,000 Based out of any CHWC office.  Will require day travel withing NY State, utilizing an agency vehicle

Position Summary: The Wraparound and Special Services Director will be a dynamic, engaging, and creative person who will lead and support teams across our service areas to develop, implement, and support creative strength-based plans for children's and families. This is not traditional case management; it is creative solutions to complex needs.

Responsibilities:

Engage youth, caregivers, and county stakeholders who need a different approach to serve youth with complex needs, utilizing the strengths of the youth, caregivers, community, and the entirety of the Children's Home service delivery continuum

Plan and facilitate meetings; identify needs; develop, implement, and support customized strength-based plans; and coordinate a team of support, utilizing creative and persistent problem solving

Monitors the progression, efficacy, and on-going needs of the youth and their families. Remain responsive to the evolving needs, providing ongoing guidance and coordination of services, advocacy, and connection to resources

Engage, collaborate, and coordinate with a variety of individuals, external stakeholders, and internal teams, while leading solution focused plans that are compassionate, creative, and consistent – ensuring others "meet the person, not their file"

Lead, model, and train others in the planning process.

Complete notes, documentation, and reports in a timely manner.

Requirements Education:

Bachelor's Degree in human services or related field required

Experience:

2 years of experience in foster care, child welfare, or case management required

Driver's License and ability to maintain insurability throughout employment required
